SI (sacroiliac) joint pain refers to discomfort or dysfunction in the sacroiliac joints, which connect the sacrum at the base of the spine to the pelvis. These joints absorb shock between the upper body and legs and aid weight-bearing activities. SI joint pain typically presents as pain in the lower back, buttocks, or thighs.

  • Trauma or injury from falls, accidents, or direct blows to the pelvis.
  • Pregnancy: relaxin loosens ligaments, causing instability.
  • Degenerative joint disease and osteoarthritis with aging.

  • Infection (rarely), such as septic arthritis.

  • Painful sitting, especially on hard surfaces.
  • Radiating leg pain that typically does not cross the knee.
